A goal of the Health Planning Council is to educate the community about good chronic disease management.

National Diabetes Prevention Program (NDPP) is a CDC recognized program specifically for adults who are at risk of developing Type 2 diabetes. NDPP is a full year lifestyle change program. The program is currently in the pilot phase in Tompkins County.
