The Human Services Coalition (HSC) is in partnership with Cayuga Health System (CHS) as part of a five-year, grant funded healthcare transformation project. The overarching purpose of this project is to establish a more integrated, patient-centered and financially sustainable healthcare delivery system in Tompkins County that reduces socioeconomic and racial disparities and advances health equity by integrating social care and behavioral care into clinical practice. The Community Integration Manager will support HSC’s role in this project by recruiting, onboarding and supporting local Community Based Organizations (CBOs) as part of an integrated social care network.

About the Position:
The Community Integration Manager will contribute to the planning, implementation, monitoring, and evaluation of a five-year, grant-funded healthcare transformation project, in coordination with CHS. This position will facilitate the development of an integrated social care network between clinical and non-clinical providers, focusing on human services organizations and other providers that address social care needs.
Duties:
- Coordinate and collaborate with CHS and CBOs in the development of an integrated social care network.
- Recruit and onboard the CBOs for the social care network.
- Provide support and training opportunities for CBOs participating in the social care network.
- Conduct outreach and presentations on the progress of the integrated social care network including preparing reports and contributing to grant-related data collection activities.
- Contribute to the healthcare transformation project implementation, monitor deliverables, and ensure project milestones are met.
Qualifications:
- Bachelor’s Degree in Human Services, Healthcare, Public Administration, or related field, or comparable experience without a degree.
- Demonstrated knowledge of the not-for-profit sector, human or social services program delivery, and/or social care needs.
- Two or more years of experience in human or social service program coordination, project management, or development.
- An understanding of social determinants of health, health equity, and the relationship of social factors to health access and health outcomes.
- Commitment to creating equitable and inclusive systems of care.
- Ability to work with multiple stakeholder groups effectively, including community-based organizations, clinical providers, clients/patients, and individuals with lived experience.
Reports to: Director of Health Planning
Pay Rate: $30-$32/hour. Non-exempt, full-time, 37.5 hours per week. This is a multi-year, grant-funded position through approximately March 2028.

Duties and Responsibilities:
Network Development
- Serve as the social care lead for the network, working in partnership with a clinical care lead housed within Cayuga Health System (CHS).
- Working in coordination with CHS and other stakeholders, help to develop the integrated social care network, including roles and responsibilities, functions, capabilities, and improvements.
- Contribute to the development of a network governance model, including policies, procedures, and advisory group recruitment, incorporating principles of health equity at all levels.
- Participate in systems design, data infrastructure, and referral management system improvements. Help to evaluate and recommend appropriate technology solutions to meet network needs, including assessing off-the-shelf and custom-designed options.
- Contribute to efforts to identify unmet community needs and develop plans to address appropriate needs through this initiative.
- Identify data needs for referral management and care coordination.
Community-Based Organization Recruitment and Support
- Develop and facilitate a recruitment and onboarding process for network participant organizations, focusing on social care providers (including not-for-profit and governmental organizations).
- Recruit and onboard participant organization cohorts based upon established eligibility criteria.
- Provide training for care providers on data entry, referral systems, demographic collection, and other aspects of the systems developed.
- Facilitate testing and launch of the technical systems developed, and assist participant organizations in using these systems. Recommend modifications resulting from testing and participant usage.
- Identify competencies and facilitate the provision of assistance to meet those competencies.
- Support individual organizations participating in the network.
- Establish and support workgroups to enhance collaboration and problem-solving.
- Pilot the development of a community health provider network, made up of community health workers, advocates, and navigators to serve as initial responders for social care need referrals.
Outreach and Reporting
- Conduct outreach, provide public education, deliver presentations, and represent the initiative one-on-one with providers, in group settings, or with existing coalitions, committees, or partnerships.
- Prepare reports or other status updates as required by the funder, by Cayuga Health System, or other stakeholders.
- Contribute to grant-related data collection activities as needed.
Project Management and Other Activities
- Use project management tools and principles to manage the healthcare transformation project implementation, monitor deliverables, and ensure project milestones are met.
- Collaborate with other Human Services Coalition staff, Cayuga Health System partners, participating social care providers, and other stakeholders to meet project goals.
- Maintain accurate and complete work files, including records, documentation, and other materials.
- Participate in Human Services Coalition activities and planning, including organization-wide and Health Planning Council program-specific tasks.
- Carry out other activities or responsibilities as assigned.
Required Qualifications:
- Bachelor’s Degree in Human Services, Healthcare, Public Administration, or related fields, or comparable experience without a degree.
