Long Term Care Committee Meeting

Tompkins County Long Term Care Committee is a subcommittee of the Health Planning Council.
We will meet on Feb 6th at noon for our quarterly meeting that will function as a working meeting. All welcome to attend.
We will focus on two important issues in Tompkins County – Discharge Planning and Falls Prevention.
If you or your organization are working on efforts in Tompkins County to address issues related to falls or discharge from health organizations or would like to get involved, please attend or send Karen Brown at kbrown@hsctc.org an email.

- Identify current efforts and gaps.

- Getting the right people around the table
