We are looking for an enthusiastic and motivated person to join the Human Services Coalition of Tompkins team as a CoC Housing Specialist and Coordinated Entry Lead.
The Continuum of Care (CoC) Housing Specialist and Coordinated Entry (CE) Lead position will support the work of the Continuum of Care in a range of planning and administrative activities including managing the local Coordinated Entry process, providing staff and administrative support to the full CoC and CoC committees, and data entry and data presentation using the Homeless Management Information System database. This position works closely with stakeholders and CoC/ Youth Homelessness Demonstration Funded Program (YHDP) funded projects to promote coordinated entry and increase access to services and housing for people experiencing homelessness.
Duties of the Position:
- Develop and update as needed all Coordinated Entry policy and procedure documents
- Maintain a prioritized Coordinated Entry “By-Name” list of people experiencing homelessness using the Homeless Management Information System (HMIS)
- Provide administrative support to the committees of the CoC including assisting with scheduling, agendas and note-keeping duties.
- Conduct outreach, provide public education, deliver presentations, and represent the Continuum of Care, one-on-one with providers, in group settings, or with existing coalitions, committees, or partnerships.

Qualifications:

- At least 1-3 years’ experience in homeless response systems or with similar systems of care; including lived experiences of homelessness
- Ability to facilitate meetings and group discussions
- Experience with data-collection, review and/or analysis

- Demonstrated knowledge of the not-for-profit sector, human or social services program delivery, and/or social care needs.
- Excellent presentation and public speaking skills.
Pay Rate & Benefits:
$26.00-28.00 per hour. Non-exempt, full-time, 37.5 hours per week.
In addition to wages, HSC offers a cafeteria benefit plan with employer contributions valued at approximately $600.00/month that can be used towards medical, dental and/or vision insurance, or taken as taxable income. HSC also offers a 403(b) retirement account, an Employee Assistance Program (EAP), and generous time off, with paid holidays, vacation, and sick time.
Consistent with the Human Services Coalition’s Hybrid Work Policy, a flexible hybrid schedule (in-office and remote) may be possible.

We are looking for a helpful and supportive staff member to join the 211 Helpline team. The 211 Helpline is a program of the Human Services Coalition of Tompkins County that connects people who are in need to the services that may help them.
This position will support the Human Services Coalition’s role in the Southern Tier Social Care Network. The Social Care Network is a group of community-based organizations, healthcare providers, and other partners who are working together to provide screening, navigation, and delivery of health-related social needs services to Medicaid Members (Medicaid beneficiaries). The Human Services Coalition is one of the organizations that is participating in the network.
This position will support the Human Services Coalition’s role in the Social Care Network by focusing on screening and navigation activities. This position will screen Medicaid Members to identify their social care needs using a standardized screening tool, then carry out eligibility assessments to confirm Members’ eligibility for the Social Care Network. Depending on eligibility and need, Members may be referred to Social Care Network programs or to other local, state, and federal programs.
Duties:
- Receive Medicaid Member referrals or direct inquiries, establishing connection and rapport with clients. Gain an understanding of client needs and how to support them, depending on eligibility and services available.
- Conduct social care screenings of Medicaid Members using a standardized screening tool. Record results of the screening in the Social Care Network data platform.
- Conduct eligibility assessments of Medicaid Members to determine if they qualify for additional services through the Social Care Network. Eligibility assessment may include gathering information from the Member, healthcare providers, or other sources.
- Navigate individuals to additional Social Care Network services or to non-Social Care Network services (existing local, state, and federal services), as appropriate based on eligibility and need.
- Become familiar with the Social Care Network program and the available services. Gain an understanding of the local and regional service providers that are participating in the Social Care Network, especially those serving Tompkins County.
- Receive training as a 211 Community Services Specialist, including using 211 database systems and providing information and referrals using multiple communication channels (such as phone, text message, email, chat, and in-person).
- Other duties as designated.
This position will collaborate with another open position at the Human Services Coalition that will focus on housing services available through the Social Care Network. This position is funded through Social Care Network capacity-building funds and reimbursement for Social Care Network services. The Medicaid 1115 Waiver Amendment authorizing Social Care Network services is effective until March 31, 2027. This position depends on the continued availability of funds.

This position will serve Medicaid Members with an identified housing social care need. The Housing Social Care Navigator will provide enhanced housing services including Rent/ Temporary Housing, Pre-Tenancy Services, Housing Navigation, and Utility Set-Up and Assistance. Depending on eligibility and need, Members may be referred to Social Care Network programs or to other local, state, and federal programs.
Duties:
