How Do I Get Connected?
Fill out a Questionnaire – Privately and confidentially, you will answer questions about your needs — like having enough food, a safe place to live, a way to get places, and people you can count on.
Access Resources – With your consent, an organization in our network will contact you to learn more about what you need and navigate you to available resources in our community.
Or, you can get help directly from the 211 Helpline by dialing 2.1.1 (or toll-free 1.877.211.8667) or texting your local zip code to 898211 (texting available M-F, 9am – 5pm).
Stay Connected – Our network partners will work together to quickly and efficiently connect you to ongoing support.

Information Sharing
With your permission, the Network will share your information with organizations directly involved in your care.
How will sharing benefit me?
- You will be connected to programs, services or resources that could benefit your health and well-being
- You will have fewer forms and questions to answer when you are seeking help from members of the network
Why does the Network want to share my information?
- The information that is shared will be used to determine eligibility for resources, help you access them, and coordinate services to better serve you
- When organizations can share information, they can more quickly and easily connect you to care
How will it be shared?
- Your information will be shared in electronic formats using a secure online referral website
- The website enables organizations to securely share information with participating health and social service providers
- This website and everyone who uses it must follow all state and federal privacy and security rules and regulations that govern all health information exchanges
What will be shared?
- Information shared may include programs and services you got in the past, get now, and will get in the future. This information may also include details such as:
- Your name, address, date of birth, and contact information
- Social needs such as housing, food, transportation, employment, income, and others
Community Advisory Board
Are you interested in contributing to the development and design of the Network? We’re looking for members of the community, or their caregivers, who get health or social care in Tompkins County to join our Community Advisory Board. There are many opportunities to participate including regularly scheduled board meetings, focus groups and 1:1 interviews. Fill out this interest form to get started.
