Long Term Care Committee

The Long Term Care Committee (LTC) is comprised of community members, caregivers, medical and human service providers, and professionals representing a broad array of healthcare and residential services, businesses, and programs that serve the aging population and people with disabilities. Long term care refers to the variety of services to meet an individual’s health and personal care needs, over a short or long period of time. Services are provided in different settings including the home, community, or a facility.

The LTC Committee is the designated Long Term Care Council for Tompkins County Office for the Aging and New York Connects Program. As such, it plays a vital role in the local planning process that identifies needs of aging residents, people with disabilities, and people with long term care needs.

Tough Stuff Subcommittee

This quarterly subcommittee meeting is brought to you as part of the Long Term Care Committee, a committee of the Tompkins Health Planning Council, led by the Human Services Coalition of Tompkins County. This is a lunch hour education and conversation on special topics relating to aging, chronic illness, serious illness, death, dying, long-term care and more!  Recommendations for future speakers for LTCC and the Tough Stuff subcommittee are welcome.

Community Health Conversations

Community Health Conversations is a quarterly community learning convening that connects health care providers, human and social service organizations, and residents to share information, increase awareness of health and social needs, and promote collaborative action to improve well-being in Tompkins County. Through dialogue and education, the series helps deepen understanding of social determinants of health and strengthens partnerships that advance health equity and community health.  It does this by:
  • Promotes the sharing of information, data, and emerging trends related to health, health outcomes, and health service utilization in Tompkins County.
  • Creates opportunities for cross-sector partners to better understand community needs and coordinate efforts to improve health and well-being.
  • Increases awareness of programs, resources, and initiatives that support healthier individuals and communities.
  • Highlights the impact of social determinants of health—including housing, food access, transportation, education, and social connection—on overall well-being.
  • Encourages community engagement and participation in activities that advance health equity and improve quality of life.
  • Provides a space for learning, dialogue, and relationship-building among organizations working to address complex health and social challenges.
  • Strengthens the community’s ability to identify priorities, share promising practices, and respond to emerging health issues.
  • Advances a shared vision of healthier, safer, and more connected communities through collaboration and informed action.
